snmp plugin: HostOID option implementation

+ Allow to use `TypeInstanceOID` and `PluginInstanceOID` together.
+ Added `TypeInstancePrefix`, `PluginInstancePrefix` and `HostPrefix` options.
* Option `InstancePrefix` marked as deprecated.
+ Added documentation for new feature.
